Space for Your Own Symptoms
The checklist also leaves room for four write-in symptoms unique to your experience, so anything the standard list does not capture still makes it onto the record.
A Note on Completing Both Columns
Fill in the Prior to Injury column even when it stays mostly empty. A largely blank column is itself part of the picture, because it documents how much changed after the event.
